Step 1: Go to your PETspot Dashboard

Step 2: Select "Applications" from the menu

Step 3: Select Applications (below "Receive Applications" and above "Expressions of Interests"

Step 4: Select "Application Settings" at the top

Step 5: Select "Edit Puppy Application Form"

Step 6: Scroll to the bottom and click the GREEN button "Add new question" and type your question in the box
